To whom it may concern: this letter of authority relates to all policies and/or agreements and/or bank accounts that I/we have with your company throughout my/our complete history. I/we have appointed *******Ltd, trading as Paxton Willis, authorisation number FRN: *****, of Paxton Willis, Paxton Willis House, Bolton, Greater Manchester, BL3 *** (“Paxton Willis”), to act as my/our sole representative on my/our behalf for the purpose of submitting a Data Subject Access Request. I/we have authorised Paxton Willis to request and obtain my/our complete file of papers including copies of any policy documents, statements of account, fact find and any correspondence or voice recordings relating to the sale of all my/our policies and all of my/our agreements. Please note that all ensuing correspondence in relation to this request should be sent directly to Paxton Willis and not to me/us. Failure to adhere to this mandate may result in a formal complaint being made to the Information Commissions Office and/or British Bankers Association for failing to abide by section 11.1 of the Banking Code and/or Data Protection Act 2018. This authority will endure until further notice.

I/We further authorise you to release to Paxton Willis (“the Company”) any information, whether deemed confidential or otherwise, as may be requested from time to time by the Company, by the telephone or in writing (including by fax or e-mail), and to do so without delay. 2. I/We confirm that we have lawfully contracted with the Company and have expressly consented that all communications in relation to the matters above from you must be made direct to the Company with a copy of such communications being sent to me/us. 3. I/We confirm that any payments should be paid direct to Paxton Willis.
